Caregiver Toolkit

 

The VMP Team understands that your role as a family caregiver can be both rewarding and challenging. We are here to provide you with education, support and referrals to service. 

By enrolling in the VMP, you are helping us gather essential data about caregivers and people living with dementia in Virginia. You can also request help from our social worker. She can help you with referrals and education related to:

  • caregiver support
  • finding a doctor
  • assessing your memory
  • providing education
  • advance care planning.

Enroll in the VMP

The VMP hosts a program called Caregiver Conversations every few months. This is an interactive, virtual program where caregivers learn what kind of care they should expect from their loved ones’ doctors, what kinds of questions to ask and how to ask them. When you enroll in the VMP, you can request that information about this program be sent to you. You may also contact Kim Ivey to request information about the next program date.
